What Happened

This began with concerns about care. It became a struggle to obtain a clear account of what happened.

My concerns included the misuse of restraints and the response when I questioned that treatment. I wanted the decisions explained, the records examined and the people responsible for oversight to address what I was raising.

Stripped of dignity

Seeing a loved one physically restrained brought home how vulnerable a patient can be.

My father was a person with rights. He was never merely a task to manage. The loss of freedom and control over his own body deserved to be taken seriously.

Questions that deserved an answer

  • What was the reason for the intervention?
  • Who assessed the situation and authorized the decision?
  • What was documented, and when?
  • How was the patient monitored?
  • What was explained to the family?
  • How were the concerns investigated after they were reported?

These questions go to the treatment of a person who depended on others for care. They deserve direct answers supported by the available records.

For clarity-

There was no documentation, no doctor's orders, no reason, no notice to family,  and more importantly-

no reason to tie a man to his bed that was cognitive and compliant.

The ICU manager’s conduct demands accountability

When I raised concerns about my father’s care, I expected the ICU manager to investigate, check the records and provide honest answers. That responsibility came with her position.

Instead, I have had to challenge her own accounts while continuing to pursue answers about my father’s treatment.

Her statements repeatedly conflict with what I personally witnessed and with the records available to me. Those contradictions put her credibility directly in question.

She needs to answer for her own words. I expect her to identify what supports her statements and explain how she reconciles them with the evidence that contradicts them. Every contradiction I have identified deserves a direct response.

The person responsible for addressing concerns about her staff must also answer questions about her own conduct. Accountability includes her.

Her position carries responsibility for the accuracy of her statements and the way she handled our concerns. A management title is no substitute for evidence. Her account must withstand scrutiny.

I am angry that my family has had to spend so much time challenging inaccurate accounts about us while fundamental questions about my father’s care remain unanswered. Having to defend our own credibility has added further distress to an already devastating experience.

Discrediting my family does not answer our evidence. Her own words and conduct deserve examination.

I expect the inaccuracies I have identified to be corrected, the contradictions to be explained and our concerns to be answered. The discomfort of facing scrutiny does not remove her responsibility to account for her statements.

My father deserved dignity. Our family deserved honesty. I will continue to challenge her statements and pursue accountability through the available complaint and review processes. Her conduct remains central to the answers I am seeking.

Then came the process

I sought records, raised formal concerns, preserved communications and pursued the available complaint and review routes. Each stage brought more documents to organize, more instructions to understand and more waiting.

The work has continued alongside grief. I have had to reconstruct events while also trying to understand how the institutions examining those events reached their conclusions.

The original questions still matter. The difficulty of pursuing them deserves scrutiny too.
